Transitional Support
Includem's Transitional Support Programme is a development of the Relapse Prevention Support Service. This is being part-funded by the Big Lottery Fund from April 2009 until March 2012.
This programme assists young people, aged 16-24, who have been involved in the youth criminal justice system, to make a positive transition from adolescence into adulthood and prevent them from relapsing into offending.
Partnership working with statutory and voluntary agencies will increase young people's opportunities to enter employment and education/training.
Staff in each area have a comprehensive understanding of how to access local services, including statutory and other voluntary services.
The project will provide an advisory service to young people to access services that are already available to them, and also those which they had not been accessing e.g. housing providers, health, education and employment support services.
